Observation

Child PB's immunization record was observed to be outdated at the time of inspection. Two children were recently enrolled (12/24/2024) in provider's child care without a written proof they are scehduled to be immunized or parent/guardians providers a signed and dated statement detailing when the children's immunizations will be brought up to date and stating they understand their children will be excluded from care if the immunizations are not completed within 30 days of the specified due date, at the time of inspection.
